These Terms & Conditions ("Terms") govern your use of services provided by Holbrock Estates at Ruxton Avenue. By using our services, website, or signing agreements, you agree to these Terms.
1. Services Provided
Holbrock Estates provides assisted living services, including housing, personal care, medication supervision, social and wellness programs, and other services as agreed.
Services and amenities are described during admissions; any additional services will be agreed in writing and may involve additional fees.
2. Eligibility & Admission
- Residents must meet the license requirements for our facility (8-bed, semi-private rooms, etc.).
- We require health and functional assessments to determine whether we can safely meet your care needs.
- We do not accept Medicaid Waiver at this location.
3. Resident Responsibilities
Residents and families will:
- Provide accurate, up-to-date information (medical, contact, payment)
- Follow care plans and cooperate with caregivers
- Respect facility rules and other residents' rights and privacy
- Pay fees on time
4. Fees, Payment & Refunds
- Fees are outlined in the resident agreement and cover base services as agreed (care, meals, room).
- Any extra or optional services will be charged separately.
- Refund or adjustment policies for departure, discharge, or non-use will be described in the resident agreement.
5. Privacy & Confidentiality
- We will maintain confidentiality of resident information in compliance with relevant Maryland laws and regulations.
- Access to residents' health records is limited to those directly involved in care, or as required by law.
- Residents have rights to visitation, privacy in personal care, and to send/receive mail without undue interference.
6. Liability & Disclaimers
- While we take reasonable steps to ensure safety and quality, we are not liable for damages beyond what is required under Maryland law.
- We are not responsible for harm arising from your failure to follow care instructions, non-disclosure of health information, or from risks inherent in assisted living.
7. Termination & Discharge
We may terminate services or discharge a resident in some circumstances, including:
- If the resident's care needs exceed those for which we are licensed
- Non-payment
- Violation of policies or disruptive behavior
- Mutual agreement
Notice will be provided in writing when possible.
8. Governing Law & Dispute Resolution
These Terms are governed by the laws of the State of Maryland. Any dispute arising under these Terms will be resolved in the appropriate Maryland State courts.
9. Changes to Terms
We reserve the right to make changes to these Terms. When changes are made, we will post updated Terms on our website or provide notification to residents/families. Continued use of services after the effective date indicates acceptance.
